I got some great suggestions here and from other groups to which I belong. To tell the truth, the best name seems to be the straightforward (and therefore, boring) one: Adoption Profile, Lifebook, and Scrapbook Services

I am so glad that I journaled our meeting with S and her family, as I had completely forgotten most of it. It was eclipsed by the hospital, which is too bad. I’m thinking I should send the entry to S; maybe she’ll find it amusing.
