His circadian rhythms are all off, he'll be insomniac for days and then crash, he's tired all the time.

He has a severe concussion so he doesn't know the lengths Sam goes to to keep him alive. Sam himself is battling a busted leg that gets infected. When Dean awakens in the hospital, he now learns that Sam's fighting for his life all because he was trying to save Dean's. Cue really sick, weak Sam and guilty/protective Dean.
Lilac, written by tattooeddevil: Sam always gets down in the dumps in June, and one night Dean wants to know why and won't take no for an answer. Maybe he has to get Sam drunk to get him to talk. Turns out, Jess was pregnant when she was killed and their baby would have been born in June. Even though she was only a few weeks along, Sam had really been excited about fatherhood and every year he mourns for the child he never got to have. Dean comforts Sam while he grieves.
Of Little Brothers and Pointy Things, written by mamapranayama: Without a hospital nearby, Dean is forced to carry out some emergency surgery on Sam.
True Colors, written by annie46: After Hell, Sam is pretty screwed up. Drawing calms him down. Dean secretly saves the pictures. Or maybe it's a curtainfic and he puts them on the fridge proud-parent-style.
Never Over, written by familybizness: (Warning: descriptions of past non-con, torture) After he gets out of the hospital, Sam still has issues over Hell. He and Dean have a long overdue conversation about the sexual assault and torture that Sam went through in Hell.

Inside, Dean leads him to a mama cat trying in vain to reign in her three kittens. Dean says "pick one." Which is of course impractical, they can't have an animal cooped up in the car for days at a time. It takes Sam a little while to get what Dean's really saying.

Unless there's demon blood involved, they're painful, useless, and an all around massive pain in the tookus. Sam is frustrated that he keeps getting bouts of migraines for weeks at a time while he gets orbed out of digital photographs or commands cockroaches or involuntarily burns the socks off his feet, and Dean reassures him that it's okay, by bringing him ice packs and making fun of him.

So with no chance that Dean will save the world from his brother, Sam decides to take care of that himself. Dean finds him in time.

Normally, Sam wouldn't be too mad -- shit like that happens -- he just wishes it could have happened on a day where he didn't have to walk five miles back to their motel during a blinding snow storm -- he also wishes he had thought to bring a cell phone, a thicker coat, warmer boots, a pair of gloves and a clue as to how to find his way back because now that the snow is falling so heavily, he can't see where he is going -- getting more and more lost by the minute...
Run Your Fingers Through My Soul, by queerly_it_is: Sam/Cas, NC-17. Soul-fisting, round two. Or three. IDK Sam could be freaking out about Lucifer maybe not being really gone from him or he could require a routine check up. I don't care what the reason is; It won't take much to sell me on logistics. But soul-fisting, this time with Sam awake and aware and souled. And Cas soothing him through it and there can be awkward comfort-touches and/or Feel Better Sex.
Twenty-nine, by purplehrdwonder: It's Sam's birthday and Dean wants to celebrate. I mean, between the job, going to Hell and Castiel taking down The Wall, Sam should've been dead so many times and Dean wants to celebrate his brother still being alive for 29 years. But Sam's depressed because of all those things and the fact that he always thought that he and Jess would have married and had a couple of kids by now, don't forget that their mom was twenty-nine when she died and so was Dean. Cue awesome!BigBrother!Dean cheering Sam up with a day away from the hunt and the Leviathans. Maybe they hole up at Rufus' and they spend a day together as brothers and Dean tells Sam stories about their mom and life before YED killed her.
Lines, by thoughtyouknewr: So for some reason Dean is worried/angry/depressed and hits Sam... Again. For like, the tenth time on the show. But instead of sucking it up and keeping calm like he has before, Sam snaps and bursts into tears like a normal person would. After everything he's been through over the last 2 years he just can't handle it. Cue mega guilty and apologetic big bro.
Sad Little Boys and Puppy Dog Tails, by cherry916: Pre-teen Sam is staying with Bobby while John and Dean are on a hunt. They're longer than expected. Sam literally worries himself sick. He won't eat, or sleep, won't play outside, just hangs out by the gate in the cold waiting for the Impala, breaking Bobby's heart. But, of course keeps his feelings to himself. Unable to admit to Bobby that he's afraid his father and brother are dead.
Disappeared Among the Shady Blades by sylvia_locust: I want to see a 1st season character interact with season 7 Sam. Like, maybe Sarah Blake used her love of art to become an art therapist and runs into Sam in the mental hospital. And is shocked/horrified/grieved by what he's become.
Hope, by nblaque_impala: Curtain!fic. Sam volunteers at an animals shelter to help keep himself sane, and he always comes home to go on and on to Dean about this stupid dog he's apparently fallen in love with, making him smile more often than he has in years, so yeah, maybe Dean decided to adopt the goddamn dog already for the boy's birthday, sue him.
Conditioned Response, by killabeez: Sam trusted Cas. A lot. With his life and soul. With his brother's life and soul. So when Cas knocked the Wall down it was so completely unexpected, and now he's come to associate Cas touching him with that kind of pain. Cue Cas coming back (however AU way you do this, without him ending up incoherent in a psych ward) and seeing Sam flinch every time Cas comes near or focuses on him or anything, even during hunts when Cas is trying to heal a wound or something. And Sam forgives Cas, totally understands probably in ways no one else could, but he can't help how he reacts to him.
Solo Saxophone, by tarotgal: Dean is holed up in the motel room with a sprained ankle (or similar) while Sam has to do a salt and burn on his own. All night. In the rain. Sam comes down with one hell of a cold because of it. Dean (somewhat guiltily) frets and coddles and takes care of him.

He won't eat, or sleep, won't play outside, just hangs out by the gate in the cold waiting for the Impala, breaking Bobby's heart. But, of course keeps his feelings to himself. Unable to admit to Bobby that he's afraid his father and brother are dead.

Sam volunteers at an animals shelter to help keep himself sane, and he always comes home to go on and on to Dean about this stupid dog he's apparently fallen in love with, making him smile more often than he has in years, so yeah, maybe Dean decided to adopt the goddamn dog already for the boy's birthday, sue him.
Conditioned Response, by killabeez: Sam trusted Cas. A lot. With his life and soul. With his brother's life and soul. So when Cas knocked the Wall down it was so completely unexpected, and now he's come to associate Cas touching him with that kind of pain. Cue Cas coming back (however AU way you do this, without him ending up incoherent in a psych ward) and seeing Sam flinch every time Cas comes near or focuses on him or anything, even during hunts when Cas is trying to heal a wound or something. And Sam forgives Cas, totally understands probably in ways no one else could, but he can't help how he reacts to him.
Solo Saxophone, by tarotgal: Dean is holed up in the motel room with a sprained ankle (or similar) while Sam has to do a salt and burn on his own. All night. In the rain. Sam comes down with one hell of a cold because of it. Dean (somewhat guiltily) frets and coddles and takes care of him.
