I hope you don't mind me weighing in on this even though I'm definitely not your target group (I might be the opposite of your target group?). Just think it's interesting. For myself, I don't particularly like incest, and I don't seek out wincest, though there have certainly been a lot of wincest stories I've read and really enjoyed-- generally it's stories where it's less about the sex and more about the twisted co-dependence Sam and Dean have

For me, for incest to work, there has to be some sense of brokenness/desperation - the "us against the world" thing - but non-con is a HUGE squick of mine, so that I don't read. I mean, there's the passive-aggressive "you would if you really loved me" kind of thing, but that's part of working out the brokenness, I think.
I do think the issue of possible pregnancy does need to be addressed when the situation is m/f.
It's easy enough to write in the condom or have the girl be on the pill, or in River's case, whatever future equivalent is.

The theory isn't mine, but you could possibly argue that her brain wasn't the only thing that the government tweaked - would you want your ninja machine to have to rely on space-tampax + changes due to hormones.
I'm not a great fan of wincest, but I do read wincest fics (boy/boy or girl/girl) that interest me in other ways. I don't, however, tend to read girl/boy Wincest. One of the things I like about Dean is how he determinedly performs masculinity while being in a very feminine role in his family - there are interesting genderswap fics that examine this, but adding in the wincest is a bit much for me. In the same way, Sam being the younger sibling (if the more powerful much of the time) means that I don't like girl/boy wincest when Sam is the girl, either. I don't know why this is something I don't like - I wouldn't say it goes as far as a squick. Maybe it's because I'm the only girl in my family, with brothers, nephews and male cousins? I don't know.
I don't know that I'm your target audience for the incest question, but I'm going to weigh in anyway, because incest fic makes me very meta. :P

Isolated!! That's the word I was looking for the whole time I was writing this comment. The pair in question has to be isolated from their environment, even if they're in a place with people they know and care about, e.g. River and Simon. Brain farts are not for the win.
With Sam and Dean, they're both canonically attracted to women, so it's hard for me to see them jumping into bed together, no matter the circumstances.
Yeah, I have a lot of trouble believing Dean slash - I don't read it because it doesn't interest me, and also because I see Dean as strongly heterosexual, whereas I see Sam as more flexible, if mostly inclined towards women. For me, the gay sex is often a harder obstacle to overcome than the incest, since I do think Sam and Dean are isolated enough/broken enough that they could/would in desperation end up having sex. This is why always-a-girl!Sam really works for me, I think.
