This is nothing new, I'm just moving my omegaverse ideas from the comments section of another post to someplace where I can properly bookmark it.

What I came up with was a combination of biological and social.

All 6 genders are to some extent hermaphaditic. Being called female or male has more to do with your outward appearance, thusly:

Females: built more for endurance
Males: Strength

Females: breasts appear at puberty
Males: Appear during and after pregnancy, then reshrink after weaning.

Females: external vagina, internal testes, smaller penis/clit (FUTA)
Males: external testes, internal vagina (connected to the rectum), larger penis.

However as far as reproduction goes, that has more to do with their Alpha/beta/omega designation.

Alpha males: the female parts of their genetalia are non-functional or absent. Male parts are larger than usual, including the knot. Sociologically: since they never get pregnant, they have been historically designated to fighting/leadership roles (yeah sexist). They are also (still) generally looked on as superior than the others.

Beta males: both male and female gentalia are functional, however they are less aroused by the smell of heat and less likely to go into heat themselves. They enjoy sex, but aren't overwhelmed by it and often mate casually without bonding. They can get pregnant, but aren't as fertile and have small families. Good workers.

Omega males: both male and female gentalia are functional, they have strong heats and are very fertile and are considered distracting until they've bonded (usually with an Alpha). Normally they are tied down with pregnancy and birth and aren't considered that workers. Rarest of all the male sexes.

Alpha females: Both genitalia are fully functioning, however heats are infrequent and they are the least fertile, most virile of the females. Penises tend to be as large as males. They often take leadership roles and are generally respected. Society tends to be biased towards them as managers.

Beta females: similar to beta males, fully functional but not as virile, or fertile. In addition their penises tend to be smaller, with smaller knots. Like their male counterparts, they tend to form relationships without bonds. Society treats them as workers, pretty much indistinguishably from male betas.

Omega females: Male genitalia is missing, undeveloped, they go into heats frequently and are very fertile. Generally mate bond young with an alpha, and spend their lives raising children. Like Omega males, they are the least frequent of the groups.

But now, thanks to the miracle of chemistry, Omega males and females have been leaving the world of endless childbearing to join the workforce. Alphas males and females have lost a good deal of power, as birth control has pretty much become the norm, and as many betas of both genders have pointed out, the size of one's knot has nothing to do with the wisdom of their choices.

Why six genders instead of four?: In this universe, there has to be at least 3 genders: Alpha, beta and Omega, because that's the way it was set up. With those three genders you can have all the gender tension and conflict and the oodles of delicious angst that comes with distinct sex roles, while still dealing with a m/m pairing.

But where does that put the 4th gender: females? Biologically they've been rendered redundant unless a case can be made for them.

The reason I came up with is that females do better during austere times. Their higher body fat, smaller size, better endurance would mean that even if all or most of the males died out during a famine, the females can reproduce amongst themselves and produce the next generation without them.

Similarly, in times when male attributes are more needed, say wars, the men can reproduce even if there are no/few women left.

Similarly fertility can be controlled to a degree based on how rich the resources are. In times of abundance, betas also reproduce, in times of austerity, betas become infertile, but remain strong themselves to help support and feed the children birthed by omegas.

Also, from a practical point of view, this allows characters like Sally and Sarah to exist without having to turn them into men.

Also, also, it's more symmetrical.

Also, also, also, it allows for the exploration of women as being the inseminators and not just the designated bearers of the next generation. This can lead lots of interesting places.

How does babby come out? Assbirth all the way, baby! Here's my case:

A) that's the way the kid got up there in the first place. Having two vaginas, one for insemination and the other for birth is clumsy. Belly bursting seems a bit gory. C-sections aren't exactly practical.

B) If I'm not going to assbirth, then the whole self-lubricating asshole that facilitates lots of knotty, naughty buttsex doesn't make nearly as much sense. We can't get rid of that. It's the best part!

C) If I'm going mpreg and penis knots, I might as well go whole hog. What's the fear? I think anyone venturing into this verse has a pretty cast iron stomach for squick anyway.

So the main objections to assbirthing seem to be thus:

A) It's unnatural. Not in this universe! It's perfectly natural.

B) Ouch. However I can say with authority that vaginal birth is definitely ouch too. The body is pretty darned stretchy especially in an AU verse with self-lubricating assholes.

C) But, but, but poop -- unhygienic -- infection! Obviously, for this to work, the body would have to have some natural defenses against getting poop up the womb. My thought on the subject is that it's one or the other in the rectum at a time. It would hardly be the only part doing double duty (pun somewhat unintentional).

A two pronged approach would be that, prior to going into heat and the internal vagina opening up, the colon flushes out and the person loses appetite. Thus, the way is clear when it's penis time. Secondly, the immune system for the vagina is such that it can deal with trace amounts of poop without infection, perhaps using gravity and secretions to flush out any that accidentally get up there. Since this is a natural function of the body, it would have evolved to cope.

During birth, appetite would also be suppressed and the path up the colon blocked off by swelling/muscle, that, along with the angle of the internal vagina would leave the child only one outlet.
