brief unconsciousness followed by having to sit things out for a few hours. ie, a wee bit of h/c altho with the world's least explicit c. maybe the questions i should be asking are how bad would the concussion be, what are the symptoms of same, how would you treat it, and how long until you're good to go.
in a pinch i'll totally settle for "knocked down but not out, made to sit for a bit just to be on the safe side", with no unconsciousness.
If you get knocked out cold even for a few seconds, unless you're a superhero you are gonna need medical attention. Headache for sure, possible nausea, confusion, sleepiness. Treatment is some kind of scan of the head, and then observation. No being alone for at least 24 hours.
If you want to tone it down from that, you should probably go with the down but not out. You can get a concussion without being knocked unconscious (I've had two! Oh joy!) but loss of consciousness is the big medical trigger- that's what they always ask first if you come in with any kind of head injury.
then i am definitely going with down but not out, because a hospital trip is a little more h than i was planning on. (also the fic i need to know this for is set in 1937, and i don't want to have to research the state of brain scanning equipment in the late 30s.)

i know nothing about concussions except that they can be dangerous, and since i don't want to introduce quite that kind of danger to the story, i've opted for "knocked onto one's ass but not knocked out, with bonus headache and nausea". much easier to deal with, way fewer consequences that would unnecessarily complicate the plot. but now i know more about concussions and unconsciousness than i did a couple days ago....
Nausea's one of the symptoms, I think - certainly one of the "whoops you go to hospital RIGHT NOW" symptoms, I was carted off in an ambulance more than once for heaving after hitting my head.

If there is Loss of Conciousness you always do have a concussion, however, you can get concussion without LOC. As a rule of thumb, the longer your out, the more severe the concussion. However, the severity of concussion varies from very little more than a head ache and nausea to confusional states, double vision, vomiting. With traumatic LOC you always have antegrad amnisia, but normally very brief. Again, the longer the antegrad amnisia lasts, the more sever the MTBI. In '37 admission would have been rather unlikely, unless it's really a sever head wound or he's very ill - so you have much more to play with.... glad it was still useful, so all that typing was not in vain....
there's gotta be an easier way to paint a tardis nail. i mean, the thing is practically nail-shaped already. if you do figure it out, you'll have to share the evidence. :D
