OK, I may be making an incorrect statement here but from what I can remember estrogen redistributes fat to help create breasts. It also moves it to the face and bottom. If I'm correct.
Now, based on this information the fact I haven't lost weight but have been told on numerous occasion I have got slimmer, would be make that statement correct.

nuts like peanuts, brazilnuts ect are good fats most nuts will help gain weight but be good fats.

Avacardo's again good fats.

But really i think if you are someone who can't gain weight 6 small meals in the day instead of 2-3 bigger meals is a better idea!

Full fat milkshakes are good for you and will help you gain weight too.

Buy a 10 pound bucket of lard and eat it over a week. Keep doing this for a year and you should be good.

On a more serious note, you can likely get 20 different replies to this post, and all will be correct.

My advice? Eat when you're hungry, and only when you're hungry. Don't overeat any one meal, but maybe try to add another (small) meal to the menu. Very little saturated fats, stick to the unsaturated fats, as a big problem with adding extra fat to the menu is raising your LDL, without also increasing your HDL. I've always had high cholesterol (genetic), but my HDL is always very high so I've had no concern.

One of the biggest problems for people who are underweight or overweight is irregular dietary habits. Going long hours without food is not good for you, as is overeating in a single meal. Always eat breakfast. Space other meals no more than 6 hours apart. Smaller meals about every 4 hours is actually very healthy for you.

If you aren't in the habit of eating much then it might take you a bit to increase your appetite to eat more meals. But for best effect you probably want to be working on eating more regularly and hence more often as well as adding fat to what you eat. Go for healthy fats- nuts and avardos as has already been said, remember nut butters too. Also olive oil or some other good cooking oild instead of those 1cal sprays. Dairy products have bad fats but soy have good so it might be worth switching to soy milk if you are going to significantly increase your dairy intake. Olives and oily fish too.

You could start taking your calories with something like myplate too. Don't do it every day because obsessing over food isn't healthy. But use it to set a goal, check yourself against it, make some adjustments then check a again. It'll help you learn more about the nutrition of what you eat while you are at it. Try doing things like keeping some nuts on you to pick at through the day as well

I think you should ask for nutrition advice in a more diet-focused forum, mostly because it's unlikely that anyone here is an expert on gaining weight; there are definitely cis-women with the same issue, so you could ask your question without disclosing your trans status elsewhere. Or, better yet, if you can, ask an actual doctor for advice about healthy ways to gain fat! Many thanks for the suggestion.

I have already approached the NHS about specialist advice and have asked in several places on the internet for their thoughts and ideas.

I do realize that this forum isn't a one stop shop of advice.

I don't think you'd need more fat, actually; just give it time. ...speaking as a trans guy who has been severely underweight all his life and still managed to get fat in areas he did not want fat in.
