The correct wording is straight from the Constitution:
"I do solemnly swear (or affirm) that I will faithfully execute the office of president of the United States, and will to the best of my ability, preserve, protect and defend the constitution of the United States."
(Most presidents have added their name to the front and a "so help me God" to the end.)

I would have thought the Chief Justice would have had a card to read from or something like that.
