And I'm sorry about your sister! That happened to my mom once when we were on a paid trail ride. Her horse just did not get along with one of the other horses. It reared up, and the pommel went straight into Mom's stomach before she fell off. Left a giant bruise on her stomach that ended up leaving a knot of scar tissue.
