I haven't watched the anime in so long… almost a year, some say.
I'm really curious about this because I thought honorifics (or those name attachment things idek and I'm supposed to be a linguistics major) were standard. Korean has the same thing, and while it's possible to drop them, I mean… I've never thought to? So I wonder about whether this is common in Japanese or if this is a Sasuke thing. It's never talked about in canon, that's all, and it's interesting.
But he calls Itachi niisan, doesn't he?

I have actually not read the manga or seen the anime in months. I'm getting a little bored of it, tbh -- I THINK IT'S THE LACK OF NARUTO/SASUKE INTERACTION. But I can still remember the initial stuff.
I think it's a Sasuke thing. I think it's a way of distancing himself from the people around him, because if he just uses their name, then he doesn't have to be reminded of the connotations of the honorific and the connection that goes with it. Yes, he calls Itachi niisan, but that's before the deaths and Itachi going rogue ( ... )

I have this one Original Story where I couldn't decide if the main character was a boy or a girl, and since I was writing it in First Person, I didn't have to decide right away. Yet I kept writing not knowing what gender or sex the character was. Neither had I decided on a name. This character was very faceless for a time (by now this conflict has been resolve though, haha).

I'm cool with thoughts just being in italics or whatever--my problem comes in when people start trying to use "--thoughts" to denote them--no quotes, because then they just blend into the narrative, and sometimes that's a useful tool, but mostly it's fucking annoying. (That's actually how speech is noted in Spanish, sometimes. It's really bothersome, fromt he perspective of someone who is used to having "this stuff" around thoughts and speech.)
