8- Gen. S4 or after: Sam gets strep in August, prompting much teasing from his brother (Who gets sick in the middle of the summer?). Then it happens again, a little worse, at Thanksgiving, then once more, a lot worse, at Christmas. Dean tries to convice his brother to see a doctor (It's not normal to spend half the year without a voice, Sam.). But Sam shrugs him off and insists they go on with business as usual. Over the next week, he slowly wears down to the point where he whispers everything, randomly sleeps away a few hours every afternoon and barely touches food. A week later, when Sam's stomach forces them to pull over on the interstate and he's still running a high fever, Dean puts his foot down, drives them to an ER where they depressingly ring in another year amongst rowdy drunks who refuse to shut up so Sam can sleep while he gets IV antibiotics into him. 13- Gen. late S1/early S2 'verse... Sam gets one of those horrible, blinding visions that leave him all pale and shaky and his head's hurting. He recovers soon enough and shakes it off, writing it off as "just a vision". But then it recurs, and recurs again... basically it becomes almost daily (or more than once a day). And each time, it becomes more violent and more debilitating, but it's always the same vision. 14- Unbeknownst to anyone, Jess' ghost has been following the boys around all the time, trying to watch over Sam but ultimately unable to do anything to help him.
At some point after Sam broke the last seal (but before he is reveled to be Lucifer's vessel) he either falls very ill or is severly injured. Either way, he is going to die. Castiel still has enough mojo to help him but hesitates, thinking that considering Sam's nature and actions it might be better, in the grand scheme of things, if he didn't make it. But then he becomes aware of Jess and she manages to make him see beyond the "boy with the demon blood". Prompt is

17- Gen. AU-post S4/apocalypse. Sam loses his hearing -- all of it. And on the top of it he has severe Tinnitus (
http://en.wikipedia.org/wiki/Tinnitus). He's always taken his hearing for granted, not realizing just how much of it is auditory-based until he loses it. Dean's awesome and there and solid and stops Sam from withdrawing completely into himself and shutting down. Prompt is

18- Gen. AU-post S3/S4-ish. Dean's back from Hell. Sam's been de-aged but retained his ginormous size. There is no hope for reversal (maybe because of something relating to his demon blood). Due to his mental age (which is complete - he has no memories of ever being an adult or anything that happened in his life... no Dad dying, no Jess, no YED, no Dean gone to Hell), people see him as "special" and Dean has to quit hunting to take care of his massively huge, five-year-old brother while grappling with his own PTS demons from Hell. The Seals are never broken (aside from the first few... namely Seal 1) and the apocalypse never happens because Sam never breaks the final Seal.
And at some point, while a child, Sam gets sick... like REALLY sick the way little kids do and he's all crying and clingy and all that stuff and Dean's stuck dealing with a huge, feverish Sam being all octopus all over him... Prompt is

20- Gen. Pre-Series. When Sam’s 15 he becomes seriously ill/seriously injured and Dean takes him to the hospital. Hospital staff, very concerned and trying to do the right thing (if you make the hospital staff evil schmucks who just don’t understand, I’ll be very sad), ask repeatedly about the father and if Dean is in fact the legal guardian of Sam. Dean can provide no documentation to prove this and, for Sam’s benefit, he’s moved to another facility to recover and then he’ll be put into a foster home by CPS. Dean’s distraught, probably hysterical, but this is Sam’s story. After a come and go recovery where everything’s blurry and bright and painful, Sam’s sent to a foster home and everything he ever wanted. Stability. Normalcy. A cute neighbor girl who doesn’t think he’s a giant freak despite what he’s just been through. A school he can stay at. He’s still weak from his ordeal, hell permanently damage him if you wanna, and everything logical tells him that he needs to stay put, keep his head down, and get stronger. His heart tells him otherwise and Sam, weakened, maybe injured, turns away from the bed that’s soft and his, and sets out to find Dean and, yes, John, too. (I think this should maybe be more than Sam calling up Dean and saying ‘come get me’ - so rule out the phones in whatever fashion you think best. I want Sam to go on a little journey here.) Prompt is

Clicky Me, Please. Sam dives into the cage with Michael/Adam and down they go. The fall is long, painful, full of accusation and fighting and torment. At the bottom is the silvery shimmer of water. Michael, Lucifer, Sam, and Adam all plunge into the river Styx. Sam eventually comes up again despite the weight on his ankle and Lucifer and Michael both trying to burn their way into (back into) his body. (Adam's just gone on. Soul spared. WTFever, I do not care.) Sam fights to reach the shore, fights to stay sane, and maybe he's not sure why, maybe he knows he deserves this and that he should just allow himself to be pulled down. But he doesn't.
Eventually, he sits on the shore of the river, listens to the creaking sound of wood over water as, oh, say a ferry draws near, and he thinks of Dean, life, and the desire to go home to both.

Trigeminal Neuralgia (tic douloureux). Which wikepedia (or Webmd or medscape, just use your google-fu) will explain in further detail. Basically, pain may be felt in any or all of the following: the ear, eye, lips, nose, scalp, forehead, cheeks, teeth, or jaw and alongside of the face, it is not easily controlled or cured. Poor Sammy. Please to be seeing this section: Wind, high pitched sounds, loud noises such as concerts or crowds, chewing, and talking can aggravate the condition, in many patients. I would love some lovely comforty fic where gets his emotions returned to him and suddenly has to actually feel everything that happened to him: Lucifer possessing him, the true depth of his manipulation through the years being revealed by Lucifer, all the possessed people Lucifer killed and drank while wearing him, beating Dean so badly, killing Castiel and Bobby, being trapped in Hell for who knows how long etc.

29- In the beginning of season 5 Sam's feeling a butt load of guilt and Dean pretty much won't have a normal conversation with him that doesn't involve "You started the apocalypse".
Pretty much alone, scared and in need of someone to talk too Sam befriends another hunter who seems to understand what hes going through and talks to him like a normal human being. Sam's not stupid through he's already been duped once and has his guard up.
But what Sam doesn't know is the hunter is part of a ring of them who want nothing more than to Torture Sam till there hearts content. Sam's expecting just having a stern talk down and maybe a few punches but he doesn't realize the length these hunters are willing to go through to tear his insides out. Prompt is

30- Future!fic, with a lot of lingering issues. Sam/Dean or Gen.
Things are OK between Sam and Dean, but not great. Then Sam ends up with some permanently disabling injury or chronic illness. He needs a lot of care, and it's not like Dean's going to shirk the job. But from Sam's point of view, he's back to being a complete burden on Dean's life. As long as Sam's around and in this state Dean can't hunt, nor can he go to Lisa or some other happy, independent existence. They are in full throttle research mode when Sam manages to drop a dictionary on his foot. A seriously heavy dictionary -- say the Oxford Latin Dictionary, since it's not online. A lot of swearing from Sam, a lot of mocking from Dean, till Dean realizes that the foot actually is broken. Pretty nastily broken, as it turns out. Followed by a trip to the ER and a few weeks of Sam on crutches, with Dean alternately being helpful and telling every witness they interview a new, embarrassing, and completely fictitious story about how Sam broke his foot. 40- Gen, please. Any season. AU but of course. Sam suffers from seizures as a result of a previous head trauma. Sam has a
seizure response dog which can be trained, in the event of a seizure, to get help, immoblize him, carry med info (the thought of Sam & said dog having matching medic alert bracelet/dog tag is kinda cute), sense the onset of a seizure, move items, block victim from walking into dangerous things if it's a partial/walking seizure, etc.
This is probably all very handy when one's misplaced one's brother during a hunt and more than protecting himself, really just wants to find him. Of course, the dog may be conflicted -lay on Sammy, keep him safe, or go get help.
I dunno. Just the thought of Sam telling the dog to go find Dean is somehow awful and cute and sweet and I'm probably touched in the head.

41- Gen, post season 5/pre-season 6. I'm not mentioning spoilers, but if you want to use info from upcoming episodes to write this, you can. It's mostly dreamy and descriptive and sad and abstract, so the logistics are only as important as you want to make them.
Between the cage and Sam waking the night of his return is a lonely, surreal time. A time for a quest, maybe. A search. Not for his brother, whom he loves and can find. But for his father, who he knows cannot be found in Heaven and whom Sam now searches out to make his peace. To forgive and be forgiven. Maybe just to tell him that he loves him, he gets it, even if he wishes it could have been different, and to say/do whatever he thinks needs to be said so that his Dad can find his heaven, his peace, and the love of his life. Make it as dreamy and creepy or sad or as unglued as you possibly can/want. Prompt is

42- Prompt: Gen. After the end of Season 5, Sam is reborn back into his life, but with his memories intact. Being an adult inside a baby's body is hard. Unable to express himself except through the most basic of actions, unable to control his motor functions, he withdraws into himself, mostly lost in thought. 43- Sam is horrifically burned during the time Dean is in hell after S3. When Dean returns it's to a brother whose face he barely recognises. Sam is still in a lot of pain and won't look in the mirror for fear of seeing the monster he knows is there.

44- Gen! S5 preferred. Generally a progressively downhill - physically and mentally - Sam fic. (Dean is tired, and barely cares anymore. Sam is guilt-ridden, but trying.) The brothers run into something- can be a witch, trickster, angel, whatever fits. In order to make Sam see the evil inside him, it curses him by taking the proverb "hear no evil, see no evil, speak no evil" literally. Nothing happens for a while, and Dean dismisses it, until they start their next hunt. At first it's little things while Sam's researching: ringing in his ears, blurry eyes, coughing. It hits full force when they go against a poltergeist (or whatever) and Sam is physically blind and deaf to it. No extreme injuries, but Sam's curse gets progressively worse, as well as his mental state (due mostly to guilt and self-doubt). Not only is he unable to research or even talk about the hunt, but he temporarily loses his sight every time he sees his reflection, goes completely mute, etc. (You can take his impairments as far as you want.) They finally go after what cursed Sam. But does the curse function on the guidelines of the one who cursed him, or Sam's own perception of evil? Prompt is

45- 6.07 & later spoilers. Sam is seriously injured while covering Dean's back on a hunt. He's clearly dying and Dean has no intention of leaving his brother's side when suddenly Tessa appears to him. She's come to reap Sam's soul, except she can't find it. She enlists Dean to help in searching for it while he tries his hardest to convince Tessa to let Sam live when they find it. Sam, meanwhile, keeps fading even though Tessa swears that neither she, nor any of the other reapers, have touched him. Whether or not they find Sam's soul and whether or not Dean makes a convincing argument is up to you. Prompt is

46- Gen or Sam/Dean; s6 AU. Sam's visions are back. Only now they are always of the same thing, Lucifer and the cage. At first he and Dean think they may just be flashbacks, but they last longer and longer, and Lucifer begins talking to Sam during them, and Cas warns them that Lucifer may be building a kind of bridgehead, finding a way through Sam's powers and the connection between them to slip out of the cage and regain his vessel. Because once Sam said Yes, he said Yes; Lucifer doesn't need his consent again. Could be that the only way to stop him is for Sam to die while he still can. Prompt is

47- Canon S4, gen. So at the end of S4, Sam was obviously struggling through withdrawal (even before Dean pulls the all-stop and locks him in Bobby's Supernatural Bomb Shelter). Sam's point of view as he struggles though the cravings, the tremors, the sweats, the whole nine yards. Feel free to take it all the way through to the Bomb Shelter scenes of LEVEE. 48- S1 or 2, Sam/Dean or gen. I don't care how they got there. I don't care how/if they get out. The boys get locked in one of those big industrial walk-in freezers with nothing more than the clothes on their backs and maybe some handwarmers if you're feeling generous.Sam took a dunk in the pool/alligator moat/communal shower/whatever, so he's not doing so hot. Dean may have been the lucky one in that he got locked in dry, but he's still not having a picnic watching Sam turn blue and frosty and incoherent. Prompt is

49- Gen, any season. Sam gets a head injury that leaves him with some serious motor-control issues -- he can't grab well, can't aim his feet well, has trouble judging distances, all that jazz. 50- After watching the pilot of The Walking Dead, I want to see a season six AU where Sam comes back from hell immediately after a zombie apocalypse. He's weak, emaciated and traumatized when he comes to in Stull Cemetery, but killing zombies and trying to find Dean - who he prays is still alive - takes his mind off everything he's been through. I'd love to see the same confusion and process of discovery that we see Rick Grimes go through in The Walking Dead (or Jim in 28 Days Later, if you haven't see the first), with Sam leaving Stull to maybe hunt for some food and water and finding deserted shops and towns. Sam leading a band of survivors, while trying to get find Dean, dealing with PTSD and with the realization that he saved the world from Lucifer, but not from everything, will all get double bonus points.
Somehow, I think Sam/Dean might make this more interesting, but I'm a gen-lover, so either way works for me. Prompt is

52- Gen or Sam/Dean. Anyone seen the BBC Mystery series Second Sight? The detective was suffering from a rare eye disease, which was slowly destroying his sight and causing distorted vision and hallucinations. (The disease was meant to be AZOOR, but a brief look online suggests that Second Sight made the hallucinations spookier and more psychological). That would be worse for a hunter, now wouldn't it? Not just losing the ability to hunt and research, bit by bit, but seeing things out of the corner of your eye, and maybe it's just the disease progressing, but maybe there IS something there. Dean doesn't know what to believe, when Sam says something is after him, something he can never quite see clearly, something Dean doesn't see at all. Prompt is

2) Everything that Sam's body went through in his real life happens again as he regrows- all the injuries, illnesses, etc. Cuts and bruises appear out of nowhere, that leg Sam broke when he was 12 snaps all over again, he gets hit with fast brutal waves of fevers and flu. It all heals really quickly, too, but Dean starts getting really freaked out at the sheer number of big and small hurts Sam suffered as a kid and teen, including some Dean never knew about (A minor injury was bigger than Sam let on? Stanford era meningitis?). Dean gets a little angsty regarding his perceived failures as a protector, and tries to make up for it this time around.

Instead of a cold, unfeeling shell he gets an insane wreck who is completely overwhelmed by the memories of centuries of neverending physical and emotional torture. Unfortunately, Dean can't deal with this on his own and eventually Sam ends up in a mental hospital, drugged and restrained to keep him from hurting himself and others in his madness. Unable to tell reality from his memories to begin with, the restrains make Sam freak out even more. Completely refusing food, plagued by horrific nightmares whenever they drug him to sleep and under constant emotional stress, he wastes away and falls sick. Castiel sticks around to help him as best he can, because he somehow feels responsible for Sam, or just because Sam is his friend. Dean can't bear to see his brother suffer like that but stays anyway because he's an awesome big brother.

66- Gen, post season 5 or 6. Sam develops Non-Hodgkin's Lymphoma and goes on treatment. Radiation and chemo. Loses his hair, Dean buys him hats, and they mostly just deal because that's how they are. But, Sam doesn't take to the chemo well and starts to get a little paranoid, starts to think that someone's poisoning him. Everyone thinks he's just depressed, has psych issues from the illness, and is maybe associating how badly the chemo makes him feel with poison or the intent of harm. Psych meds ensue and more pain meds and Sam tries repeatedly to ditch chemo, get out of it, or just rip it off. This is very hard for Dean, of course, because he knows chemo makes Sam feel like utter shit, but it's for his own good.
Except, yeah, Sam's right and he's being murdered slowly via the medication meant to save him.

Dean's pissed and a couple of months go by until somehow he finds out that Sam has been suffering alone all this time. At first Sam liked the attention/affection because he craved a mother-figure so badly. Then when things first seemed off, he figured he was just being paranoid because he didn't know how mothers were supposed to act. When the abuse crosses the line into painful and invasive, he's too embarrassed to tell, one because he let it get so far when he's supposed to be trained and tough, and two because it's a woman and Dean (maybe 13) who has started talking about girls, has teased Sam about being scared of girls because he always blushes when Dean talks. It ends up "haunting" somewhere attached to some very strong emotion, torn between the influence of the moral compass on one hand and the confusion-terror-anger on the other. --A haunting at this emotionally charged location inevitably attracts attention from certain persons. 75- Post season 5 or 6: While the relationship of Dean and Sam is once again difficult and tense, Dean decides to finally get away from his pain of a brother and leaves during one assumedly easy hunt. He doesn't leave a note, just plans to call at some point to let Sam know he hasn't been eaten by anything. But he gets distracted for a while (a girl, maybe) and during that time, Sam notices he's gone (Dean doesn't answer his calls), thinks he was taken by whatever, and goes to find and save him.
Before Dean gets a chance to get in contact with him, whatever they were hunting turns out to be a bit more dangerous that they thought. It takes Sam and tortures him for a few days or weeks (go wild here, if you want to, almost anything's fine, but if you just want to brush over it, that's fine too). 79- Gen, post BUABS. I would love to see something that deals with the traumatic fallout of being possessed in a way that parallels the aftermath of sexual assault. (Details greyed out due to potentially triggering subject matter - highlight to read).
The way I see it, having a demon take total control of your body and forcing you to do things against your will would produce reactions and emotions very similar to those associated with rape (violation, shame, anger, flashbacks, etc). Sam is traumatized by Meg's possession but is in denial, trying his best to carry on as normal and pretend nothing happened. After all, it's over and done with and Winchesters don't dwell and mope. Bonus points for Dean slowly realizing how traumatic and violating the whole experience was for Sam and being determined to help him heal. No total woobie Sam, though, please. 84- Early-ish S1. Gen. There's something hunting second-born sons and the guys decide to go and hunt it because... duh... that's what they do. Only they don't figure out the creature/fugly/witch/whatever's M/O before it latches onto Sam. Sam, who hasn't been eating, hasn't been sleeping, and is still a wreck over Jess' death goes down fast and hard. Symptoms can be anything but preferably entailing some/all of the following: crazy-high fever, night sweats, sweating, shivering, peeing pure blood, hard distended stomach, excruciating cramps, muscle spasms, seizures, being unable to keep anything down, etc. Pretty much go crazy (anything up to and including the kitchen sink is included -- but preferably stuff involving the stomach and plumbing) and make Sam weak beyond belief. 88- Gen. S2. Post-NO EXIT. A hunt that turns personal (maybe the guys couldn't save the victim, it involved kids, or had something connected with Dad... I don't care and am not fussy) goes seriously south, leaving the boys both injured and bleeding (Sam more than Dean, obviously, and it doesn't matter what their injuries are... just make them both a little bloodied-up) and grappling with "what the hell just happened?" But the thing is, Sam was already coming down with something -- preferably of the pneumonia/bronchitis variety -- when the hunt began so with his injury, he's really feeling crappy (maybe even passing out in the car). Dean's also at the end of his rope, so he hauls their butts to Harvelle Roadhouse because it's the nearest "safe" place he can think of, despite being chewed out by Ellen in NO EXIT.
Ellen's no longer seriously furious at them -- her temper burned itself out and she knows that the Winchester Boys really did do everything they could to keep Jo safe and that the events of NO EXIT wasn't entirely their faults because, hello, it's Jo, and she doesn't blame them for their father's actions involving Bill because she knows they had nothing to do with the incident -- and when she sees how beaten-to-west-hell-and-back they both are, she takes them in and tends to them in her hard-as-nails-yet-maternal way, taping up wounds, kissing their poor shattered souls, tucking them in bed and forcing them to sleep and heal, and also helping Sam through his bronchitis/pneumonia. By dinner his voice is totally gone, he's not eating, and Dean can see he's running a fever. He agrees to swallow a couple of aspirin, but he's not going to go back to the motel and lie down while the thing's still out there. So they trudge off into the woods -- and of course Sam is wearing that ridiculous light jacket meant for balmy spring nights and sneakers with no waterproofing -- and kill the thing and then Sam just collapses. Lots of getting him out of soaking, icy clothes, forcing him to swallow warm beverages, concerned forehead feelings, yelling at him for being an idiot who doesn't know how to take care of himself, and general insane levels of schmoopiness with Sam all groggy and croaky and miserable.
