It's a shame there's no other method of non hormonal contraception that doesn't involve girl part pokeage.
Make sure you ask them to warm the speculum and to use a small one, that will usually minimise the period like cramping. If you is comfortable with giving me your address, I will send you a present after you've braved the cervical smear. I won't be able to make it to your play piercing party because of health stuff (lack of spoons for travelling and not willing to voluntarily injure myself either).
The fact that you can't feel the string doesn't mean it's fallen off btw. Quite often, if the strings are left long, they will tuck themselves around your cervix and you won't be able to feel them. Or sometimes the health care professional who puts it in will do that for you, to minimise pokeage during sex.

Look, you got through the smear, and considering how you felt about it, and the issues you have, I think you did bloody well to get through it and I think you should be proud of yourself. There was a time where I'd run screaming from the building at the site of the speculum. You did well not to do that!
