Sherlock would probably notice that Sam is intelligent and tends to be led by his emotions. His thoughts will wander off. He'd be able to smell gun oil on him. He's physically fit, and has a large scar in the palm of his hand that every now and then he will play with. Sam is also a good liar, but probably not as good enough to fool Sherlock. You're in luck that I'm a sympathetic Sherlock mun here, and I'll definitely drop hints to pick up on in my posts if and when they ever end up interacting on this one. It'll probably generally be various states of disarray involving chemicals and... snacks? I don't even know what he gets up to.
The biggest thing that Sherlock is going to be a bit confused on is that the Doctor is very old, and he TALKS like someone well beyond the years he looks (Matt Smith is in his 20's, the Doctor is actually in his... 900's). It's not really something he's good at hiding, but it's not exactly something he comes immediately forward with either. He IS an alien, so there's going to be a few things about him that Sherlock isn't really going to get. He might just think the dude's plain nuts.

I'm not exactly the tv and movie sort, generally, or at least not anything made past 1940 or so, so that's definitely the exception rather than the rule). I should update that after her canon-update her harsh edges have smoothed over some. She's secure and more playful, because she's at the end of her journey and has found a home finally, rather than being in the midst of it. There's still some insecurity and confusion there, but it's less so, because her situation is better--she's no longer left hanging about things, she's just...working them out now. She's still tired, though, but it's more from her time in Arkham than before it.
Furthermore, she's not in the best health, because of the constant stress of the campus and an unhealthy dedication to studying in the library which borders on obsession. That's about all that's changed!

There might be slight traces of ink on her hands from printing stuff out for her dad, and she has a tendency to flinch the slightest bit when faced with a guy paying too much attention to her, a vestige of her rape a year ago. 2. what they make effort to conceal (if anything) and whether or not you wish that attempt to be successful She can be underhanded at times and might be a little too quick with some sentences and a little too slow with some reactions. Her dad would catch it, so I imagine Sherlock would, too. She would respect him more if he didn't advertise it, though. I'm not sure if that would matter to him. Otherwise, sure, he can expose her.
