ASG voting question

What's the rule (if there is one) if a player is voted to start the ASG, but is injured? Does the manager replace him or does the 2nd place vote getter start? Case in point: Pujols is leading 1B and has such a lead barring a drastic world wide push he will win the 1B voting, but I don't see him playing even if he does come off the DL by then. Does

It's all manager at that point. Garner could put Berkman, or Nomar, or anyone else he wants there.
