Probably not a formal interview, but be prepared for one in case it is. Follow the professor's lead for how formal the conversation will be.

I had a phone interview for a history program. I was asked questions about my research interests, past experiences, that sort of thing. I was also asked about a research paper in progress and a conference paper in progress that I mentioned in my SOP. We also talked a little bit about the program, where else I applied, the city, etc. In general, if you can talk coherently about the stuff in your application, it should be fine. Try to relax and have fun in the interview!

Yay! This is a great sign - shows that there is SOMETHING that has piqued her interest.

Yes, treat it as a formal interview. Review everything you sent in your application, reread your writing sample and sop and be ready to answer any questions that might be based on that. Maybe come up with a few additional areas you'd be interested in looking into.

I know Northwestern does phone interviews. Otherwise, I don't know.
