So nervous for annual review. I know I shouldn't be, really, because I'll be getting something from it, it's just a question of having the guts to push for what I know I deserve. It's a big jump! And I think I can be reasonably confident

Polite and assertive with lucid arguments rather than ultimatums will get you far, hon. Go get 'em - worst they can say is 'no', and if they do, quietly look for another job! ;D
