Holmes only listens for a moment to the silly deductions of the plain-clothes detective (recently left by his wife and a small step away from becoming an alcoholic), before he snorts and turns away.
So does the elderly gentleman in front of him and they knock into each other, his books falling to the ground.

While the other man bends down with a polite apology and helps picking up the books, Holmes gives him a once over (recently lost wife and baby after having been dealt a smiliar tragedy a while back and now looks more dead than alive), before falling back into his disguise as a grumpy old bookseller, he growls at the (broken) man and turns away, turning his steps towards Kensington, slowly walking up to Watson's practice.

Nothing could have prepared him for the moment when Watson lifts his head and he stares directly into the dead eyes he has seen earlier, when they bumped into each other.
Not even a thousand apologies will ever take the shame away that Holmes did not recognize Watson nor the pain knowing that he in part is responsible for the broken expression in Watson's eyes.

After three years it might be easy to drop his own disguise and be himself again, but it will take a lot more for Watson to ever look alive again.
