There's a consult in the ER for a patient with possible infectious endocarditis and rather than getting the patient in the ICU, they want me to come look at him first. What I'm expecting is somebody fat and overweight with poor dental hygiene. What I'm not expecting is a seventeen year old kid that's built like an olympic runner. I take a look at him and he's got the definite murmur, I get an echo ordered and write up orders to transfer him to the ICU. He's hypoxic and there's no doubt that he's got at least a PE going on.

The bigger risk, however, is stroke. I text Burke about the possibility of getting him in for an MVR sooner rather than later and call infectious disease myself before parking behind the counter to look over the other patients we have. At least I get to be a real doctor today.
