Yes, Dean DID call Castiel "Cas." And I laughed and laughed. Also at the "we're here to rescue you?" especially when they went back out and Dean pulled the door shut and a bit of it fell off. I laughed and laughed at them.
Otherwise, not much laughing at this episode, just a lot of "Eep!"
2. Yes, Dean called him "Cas." I was highly amused. Tickled even. It's like being on a first name basis or something.
3b. This totally reminds me of the song "Next to Lovin' (I Like Fightin') from the musical Shenandoah8. If I were a betting woman, I'd almost take it to Vegas that they are going to do an addiction storyline. They made a pretty big point of saying that when Sam uses his not-so-secret-anymore powers, his headaches are completely gone. He stops using the powers? Headaches return. He tells Ruby to take a hike (per his "I'm done with all of it), well, his pusher isn't going to take that lying down. She's gonna have to find a way to reel him back in. Otherwise her evil ulterior plan is shot. And Dean's gonna have to hold an intervention (which he's kinda already tried to do with the punches, but there's got to be a better way than that, ya know

1. I really don't want to believe that Sam is sleeping with Ruby because that? Is just wrong on many levels. But they do seem to want to give us that idea, don't they?
*nods with you* So very wrong. I could, however, see a scenario where Ruby deliberately did not reveal herself as Ruby until after she'd managed to convince Sam to sleep with her. Thus even if Sam refuses to sleep with her ever again, he's still going to be emo!Sam! and torture himself about having slept with her in the first place. (Because being Sam, he'll convince himself that he should have somehow known it was Ruby from the start.) And thus a) Ruby has leverage on him b) the other demons will pick right up on that and use it against him.
8. I gaze upon Sam's declaration that he's done toying with his Amazing Demon Sammy Powers with much skepticism. Much skepticism, indeed. *gives him incredulous eyebrow*
*nods again, a lot* I remember there was a shot in S3 of Sam reading Dr. Faustus, which was a pretty anvilicious hint that Sam will go back and forth about the

Thinking such thoughts about my dear Sam is breaking my heart.
Therefore this explanation might help save my sanity!

I am similarly heartbroken to think Sam would be deliberately--especially after his OWN experience with possession and what he watched himself doing to Jo--violating a host body in such a manner.
But from Ruby's POV, it would make sense for her to pull that nasty of a trick on an unsuspecting Sam. Especially if she wasn't sure that in his grief/rage/angst after Dean's death, he wouldn't exorcise her on sight if she revealed herself too soon. So getting close to him would have required more, er, conventional means.*wry grin*

Yes, and I think it's not a term of affection, but one of disrespect, as if he called Lucifer "Lucy" or something like that.
"9. Oh! And my favorite bit: "Um. We're here to save you, I guess?" *falls out of chair giggling*"
OMG I can't stop laughing at that little tiny bit. I keep watching it over and over. Their faces are PRICELESS, especially Sam's. :)
Overall, AMAZING episode. Can't WAIT for next week.

I was watching that bit without the sound today (was on the phone at the time) and their facial expressions are hi-larious! Hee!
