I'm also voting dead wife, just given his luck. It could also explain half of his actions.

Although from the way he spoke/was spoken of in the omiai episode, it sounded as if he'd never been married at all? That might have something to do with the parallels between the anime and the manga though.
How old is he, anyway?

I don't think so, since he doesn't seem to like his parents very much. (He says so in the anime at one point - they gave him a name that ensured his whole life would be miserable, after all.)
