I've never had a problem. I think that singers get used to scanning the dynamic markings, notes and lyrics very quickly even though they're on three different levels, the way pianists get used to reading two (or more) staves at once in different clefs and making sense out of it quickly, without constantly pausing to work it out. (And when you think about it, the melodic line and the lyrics in a vocal line are actually closer together than two piano staves.)
It's just a matter of training your eye. I mean, my sight-reading skills on the piano are TERRIBLE because I'm not used to reading music that's laid out like that. It's a little beyond me how someone can scan two staves in different clefs and instantly make enough sense out of all of it to play. But that's just because I haven't practiced it enough. Vocal lines are easy to me because I read them all the time.
