Drop totem then hit with flame shock, ignore using lighting bolt. It's useless until you get windfury and the instant cast procs. This is how I go through at that level:
Totem, flame shock, primal strike, lava lash, earth shock and by then primal would be off cool down again. I use earth shock at the end to stop the damage that happens between waiting for the cds when you are autoattacking. Leveling as enh is slow until you start getting more buttons to push. By then you'll need a macro though because there are too many lol.
Pretty much everything that anguisel said. If you get more than one mob, make sure you have flame shock on one, and then use lightning lash. It spreads the flame shock. I can't remember when you get fire nova, but that's a good one to use for multiple mobs. Magma totem works for groups, too, but it's been awhile since I had a shammy at that level, so I don't know if you have it yet.
