I didn't bother much with them at all until I got Call of the Elements. Even then, I found trash died too fast for them to be useful. I started using them a bit more in the later Vanilla dungeons and a lot of that was for mainly forflametongue and wrath of the air. Also, I used mana spring instead of healing stream up to LK content - I felt like healing stream didn't do much until then.
Whenever you can, where it makes sense. This is really vague, I know, but you can provide a handful of different buffs that can mean even the difference between a kill and a wipe, or more usually, just a faster and easier kill. You determine when you want and if it's worth the time or mana

I usually wait until after the first trash pull. If I'm having trouble keeping the tank up, if the tank is having trouble holding aggro or if there is just a lot of damage then I throw my totems down on trash otherwise I just do it on boss pulls. To be honest this is my 3rd resto shaman and I usually don't bother with totems on trash in instances until around lvl 68, when you start zoning into Northrend dungeons but gear doesn't have a lot of mitigation on it yet. You can always glyph to get more mana back on totem recall but then you need to remember to recall them :p FWIW I have them keybound to "T" with the recall button on "Shift T" And keep in mind the way totems work is changing in MoP so there will be quite a bit more guidance on how/when to use totems soon.
while leveling in a dungeon sense, it's useful to throw them down occasionally but i don't think the mana cost is totally worth it until you get closer to max

A glyphed stoneclaw totem also provides you with a damage absorb for times when you've pulled extra mobs or a tank is having trouble gathering up adds in a dungeon. At low levels, you may want to use that glyph spot for something else, but a stoneclaw absorb can definitely come in handy, especially in PvP.
