Y'know...I do have friends who still go to Burning Man, or Pennsic, and they have little ones. They don't take them, generally--the kids have a vacation with grandparents or family, or something.
The people I know in the Camberville artist/burner/spinner/freak scenes are turning into parents, and while they do curtail some of their activities, they still manage to go to parties, go rock climbing, or biking, dancing, playing with aerial silks, spinning fire, etc. They don't do it as easily, sure--they have to make some concessions, and plan more--but they find ways to include their kid (moominmolly, for instance, just got her 3 year old riding a tandem bike with her and her husband), or find things for their kid to do instead

as far as fellowships goes, i know some are more flexible. for example, when tanya did the geriatric fellowship at MDFMR, she stretched the 1 year fellowship over 18 months, essentially doing 2/3 time, so she could spend more time with her daughter. i would hope that an integrative medicine fellowship would acknowledge the importance of family time to one's wellbeing, so would strive to accomodate your needs, if you want to do something similar.
camping is probably the easiest thing to start with a baby. cottage/cabin camping would be as easy as packing a playpen/portable crib, as well as the typical baby gear. tent camping might involve a bigger tent and small travel crib, but the latter would be useful for any travel. you can also probably find the gear used or ask for it for christmas, or as baby gifts. actually, the baby carrier for a bicycle would also be a great baby gift.

As for the balance I have no worries that you will come up with the optimal situation for all 3 of you.
*Dads are really good at taking care of kids when Mom's have a career goal to complete.
*Your "crazy" activities will be modified to still make you happy and well that is one of the wonderful things about Grandparents ;)
I look forward to hearing how the pregnancy progresses and what else you learn about yourself through the experience.
