1. If you are going with the 'fired' explanation (in some continuities, Dick quit), they had been arguing a lot, and then Dick got shot. Bruce freaked out, decided it was too dangerous for him, and fired him.
2. Black Mask, and she was taken to Leslie, who supposedly refused to treat her in order to truly show Bruce the cost of his mission.
4 - As of the beginning of Countdown, Jason's days of killing seem to be mostly behind him, as evidenced by the fight between him and Dick and Tim over Duela's death. He's only killed once since then, and it was a really bad guy that resembled a fish thing somewhere in the Palmerverse/Nanoverse. So yeah, I think his days of insanity are over. :p Donna and Kyle willingly working with him should prove that he's on the good side again.
The answer to #1 will kind of depend on which person you ask and which continuity.
In the last reboot, Dick didn't get fired, he just moved on. Sometimes he quits. In one version, he got fired for not spending enough time as Robin in Gotham, as I recall. He also gets fired because he got injured (Batman "fired" him to keep him out of danger).
#2, She was after Black Mask. As for the time of death, I think you are talking about Leslie Thompkins. She is the crime alley doctor that was with Steph when Batman said the immortal words "Make a hole" and worked through the crowd to get to the room where she was dying.

Well that's just stupid! That's sacrificing needlessly!
Thank you for telling me though, and I guess now I can tell the class Dick either quit or was fired. Depending on which continuity they want to go on.
