From my experience replacing a social security card, it can all be done over the phone. Granted, that was awhile ago.

P.S. I think birth certificates only cost $20. I know, it's still $20 you have to find, but if it helps you get insurance, it seems a good price to pay. Good luck getting all that stuff done. I hate that kind of stuff. Always a thousand stupid forms to sign.

If it's Medicaid you're applying for, they will pay for the costs of getting any of the documentation they require. Call the number on the bottom of the letter and explain this and they'll help!
