the ones designed for enhancing existing breasts do a pretty decent job of creating them to. i use them. though i do have the advantage of a little excess weight to help.
I use some silicone breast forms - they're advertised as being for enhancing existing boobs but in reality they're often sold in the cross-dressing section of online stores. It's the only actually vaguely CD thing I own (If you don't count a wig!) and I can thoroughly recommend them as looking very natural and feeling comfy. They look a little small when you first get them but they sort of fit over your existing chest quite well they take me from being flat chested (Not on hormones yet) up to somewhere between an A and a B cup, which is quite enoough for me and probably about where I'll end up naturally anyway.
They do have a slight nipple on them but it's just enough to show through a tight bra and top, just like the real thing. Before that I was using home made ones and they were nowhere near as good. Here's a photo of the ones I have, or a larger picture if you prefer

... is your friend! They do cheap-enough silicone enhancers which should bring you up a size. I've been a AA and am now a B & these things have helped me along.

25 quid for a set which is a lot cheaper than paying hundreds for breast forms. Oh, and keeping them in all day is okay-ish, too.
