We don't have a standard policy. Most teachers don't exempt anyone from finals (we don't have mid-term exams). I exempt students who have an A (not A-, must be a solid A) but I'm one of the few.

Our "A" is an 89.5

Ours is a 93%, so they've got a pretty good grasp if they have it. Also, the kids with an A are also the kids who study the most and get an A on the final

My daughter was in an Academy and always had application finals. my son was public and some of his teachers would allow students skip the final if they had a 3.85 in class or could use it to replace the lowest grade.  For me, I preferred the grade replacement compared to not taking it but I do think it should be left up to the teacher. IMO

I don't hear of exam exemptions in Australia... it sounds odd.

Isn't the point of an exam to examine and confirm the students knowledge in a controlled environment and even playing field? There are many reasons students might / might not be able to do well on a research paper, so an exam paper is designed to confirm their knowledge on a sampling of what was studied...

You could also say that there's plenty of reasons a kid might not do well on a semester exam. I had a couple of resource kids who bombed mine on Tuesday - test anxiety, not taking it in the resource room but instead with 100 other kids in the gym, etc. I had a girl bomb it because she was sick - you could literally tell she felt like shit. Some kids just aren't good text takers.

Indeed there are many reasons, however sadly it's a fact of life that there will be occasions where you are not able to do the work an alternate way. Drivers Licences, university studies, university entrance exams, employment testing etc ( ... )

That seems really illogical to me. I mean, in many subjects, the term is supposed to prepare you for a summative exam, not be a barometre in and of itself. Seems strange and, like the OP said, dangerously entitled!

That is sort of my issue with the exemptions. I feel as if the kids don't keep notebooks in order or take their work seriously as they just try to pass the quarters as they come.

If they were required to take the exams perhaps they would be a bit more careful.
