This was great!

Very sad for Dean though...

I thought it would have been cool if Cas was real--like from an alternate dimension or something....but I guess he's not. It's even more heartbreaking this way--Dean's delusions are just changing and the meds he was on no longer help.

I'd love to read more.

I played with how real I wanted Cas to be--originally this was going to end with Jimmy Novak making an appearance, but I thought that made actually it too explicit that Cas was a hallucination. I had a line in there about Sam seeing the wing shadows in the kitchen but it felt forced so...Cas's realness or not realness is your call. As is the significance of Sam's dreams.

And yeah, poor fucking Dean, right? Right.

This was interesting. You did a great job of keeping it inconclusive as to whether or not Cas was real and whether or not Sam had experienced in some way the same delusion/illusion life that Dean had. Very cool.

Edit: This may sound strange, but your userhead is making me think of a ridiculously trippy and truly terrifying claymation short I saw as a young kid, interpreting Mark Twain's The Mysterious Stranger (a book I read at far too young an age, which might explain a lot). If you haven't seen it I probably sound crazy, and if you have you probably remember it cuz it was fuuuuucked up. The internet is insane and magical, so here it is in all its fucked up glory...yeah, rambling.

Poor Dean, he was so desperate. And I could feel Sam's and Jess's exhaustion.

The ambiguity at the ending was thought provoking. What if Sam's reality was the false one, and Dean was right?

I got the idea just now writing this comment that maybe this is some kind of parallel universe Dean and Sam, and because there are no ghosts or demons there, Cas has to resort of other means.

Or, you know, maybe poor Dean is just hallucinating. :(

Sort of imagining Sam going crazy now and being crazy with Dean and Jess just kinda...poor Jess. :(
