I started tracking all the books I read in 1996, in a paper journal (I have one specifically designed for logging books, but any will work). I simply write down the author, title, and dates I read it (as specifically as I can remember).

I made a decision to only record books when I read them for the first time after beginning the log (which accounts for occasional gaps when I go on rereading binges) and (for some reason that made sense to my 19-year old self) I decided not to include books I read for college classes (about which I am sorry now, just as I am sorry I did not start the log when it was first suggested to me, namely the seventh grade).

It's low tech and not that snazzy, but it seems to work for me.
