Man, it's been a long time, but I think the first time I played the game I got the Leave ending. I think probably in part cause I was playing so cautiously with my health since I had no idea what would happen next. Then lately I played it again without any particular ending in mind, and I ended up with In Water! I was kind of surprised by it, haha. I wonder how much of my prior knowledge of the jumps and scares and secrets of the game ended up influencing how I played it when I came back to it again...
I also got Leave on my first playthrough, although I was thoroughly spoiled and specifically aiming for it, because obviously I had to get the best possible outcome for MY TERRIBLE BOYFRIEND JAMES SUNDERLAND.
Like you, I was a little surprised to get In Water on a replay! I suppose, if you ignore the health aspect, the Leave and In Water endings have pretty similar requirements, and then there are a couple of things on top of those that can nudge it towards In Water: reading the diary on the hospital roof, listening to the headphones in the hotel. So, if you're a thorough player and you check everything out, I can see that that might make In Water more likely.
The suggestion that you're more likely to be cautious with your health on a first playthrough is a really interesting one, though! That hadn't occurred to me, but you're right; you've got more incentive to keep your health topped up at all times if you don't know what's around the next corner.
Haha on my recent playthrough I also had someone watching so OF COURSE I HAD TO SHOW THEM EVERYTHING which probably didn't help. Add that to playing free and loose with health all cockylike and welp, into the water James goes.
I was super vigilant my first time around! I was so nervous haha. Especially coming directly from SH1 to 2... I remember health coming at a premium in 1, so I was VERY careful with my health in 2 my first time around. As time's gone on and I've gotten more used to Silent Hill though, I think I've gotten more stingy with health and end up running low more often than not...
