Kim, you are not horrible parents. This is most likely not your fault. Parenthood is a never-ending head scratching experience. Has his teachers noticed anything? Maybe they have some resources to test him or tell you where to turn. Otherwise, his regular doctor would be good to start. Tell him or her all the stuff you posted here. That's my two cents worth.

His teachers always notice things, and I think they will all be very helpful and willing to be involved in doing what it takes to get him where he needs to be (especially his math teacher, OMG we LOVE his math teacher).
I called his health plan's member services today to ask them where and how to start. We don't need a referral for this kind of thing according to them, so I said where I'd like to take him, they told me it was perfectly okay to take him there, and gave me a phone number. The clinic is closed on Fridays, so I'll be calling them on Monday to start the ball rolling.

I'm glad you already looked at ADD so I don't have to convince you. He's textbook and EXACTLY like my own little ADD princess. They'll make you crazy but there are wonderful medications available. Don't be upset with yourself. You see that trees a problem and it can be dealt with. He just needs an appointment with a pediatrician. He's old enoug to be a little defensive or insulted so just tell him you want to help him focus. Kids are funny. Make sure you have time to talk to the dr on your own so you can be blunt. Hang in there sweetie. *hugs*
Hee. He's been drinking coffee since he was two or three years old, it doesn't phase him in the least. Sugar in general does not cause the hyperactivity you might expect
