I'm 36w6d today(woohoo!), Even though I live in Canada, I do not have easy access to a doctor in my own province and must pay out of pocket if I consult offices out of province, but hospital care is free

I'd pay a fee and go to a health clinic, since it sounds like you've had the sinus problems since before your pregnancy. I'd also write off that family dr of yours, no point in paying someone who doesn't want to make you better! Hope you get well soon love! X
It sounds like you're suffering (so why wait) and it isn't enough of an emergency for ER. I'd worry L&D or OB--well it's just not their area of expertise.
I noticed you're in Quebec and essentially going to the ER would get you automatically sent to L&D and they'd probably send you home. No emergency clinics on this side of the bridge? That's personally what I'd do (and I'm in a difficult area for those!) or call info-sante (811) they always end up telling people to consult, but they can also tell you about nearby clinics and how to get appointments.
There are a few. Unfortunately, you have to get to the office within 15 minutes of it openning to be able to see a doctor. I was able to reach the ob/gyn's office before they closed for the holiday. They referred me to the (ontario) hospital ER. A doc was able to see me and send me back home within 1h30. Makes me want to move across the river!
