to consult or not consult

I'm 36w6d today(woohoo!), Even though I live in Canada, I do not have easy access to a doctor in my own province and must pay out of pocket if I consult offices out of province, but hospital care is free.

I've suffered from nasal congestion/sinus throughout the pregnancy (and beyond). Between my 34w and 36w appointment, my ear and neck had started to ache. My OB/gyn told me to take Tylenol for discomfort and it helped for the most part.

Since last night, my neck is stiff (but still moves), I have some chest congestion and the pain doesn't go away with the Tylenol. (but sleeping seems to help out a lot). I'm having heartburns and contraction which makes me believe that baby will be here sooner than later. My next appointment is scheduled for next Thursday, when the doctor comes back from vacation.

Since I've suffered from anxiety 1.5 years ago, my family doctor has blamed all my aches and pains (mainly, sinus infections) on stress without taking the time to check if anything else could be the culprit.

Who you consult (to try to be in as good as a shape as you can be for the upcoming delivery):

Family Doctor & pay?
Wait to Deliver/Next appointment for free?
Pay a fee to go to a Health Clinic?
Go to the ER for free?
Call L&D for free?
Call the OB/Gyn to see if another doc could give me advice for free?

or just sleep it off for a few more days?

ETA: posted to this comm as I'm not used to having access to so many doctors, I'm not sure which one to see anymore! great problem to have! :S

ETA-2:
Called the Healthline, then as per the info provided called the L&D, who asked me to call the OB/gyn office, who told me to talk to a pharmacist or go to the ER for some help as they were closing for the week (OB/gyn office is at the hospital). Pharmacist told me to talk to a doctor, so spent a little bit over an hour at the ER to find out I had an ear infection and switch to Advil to take care of the pain AND the inflammation.
