After perusing google, I would suggest the correct answer is E.

Dating the patient is clearly not suggested, and continuing to treat the patient when it is clear they have an interest may make certain situations awkward. Likewise referring the patient to a colleague could cause conflicts, so E, neither continuing to treat nor seeing the patient socially would be the safest possible route.

....In practice I imagine it is option Aa - Begin to see the patient socially and just don't tell anyone.

Possibly not the best answer if you are being tested.
