Such as - that John is the one who commits the actual killings because he's had special surgical training, so he knows what the Old Ones' vulnerable points are. It helps explain why it's so hard for humans to fight against them, as a delay in seeking for vulnerable points would be fatal, and also why Sherlock in the original story had to decoy the victim for so long, instead of doing it himself.
