It was a combo of the paparazzi, DUI on her driver's part, and nobody wearing seatbelts. He was trying to outrun the paps who were in chase so was going double the speed limit for the tunnel (which was an accident black spot at the time, though I think after her death they put various other safety measures in) and he was drunk mixed on top of anti-depressants. He clipped another car switching lanes, lost control and had a head on collision with a pillar. At that speed the car didn't just stop, it ricocheted around in a way that caused other impacts.
While it can't be guaranteed seatbelts would have saved everyone, it would certainly have made the injuries much less severe and more survivable. I'm pretty sure that if anybody wanted to assassinate her they had more reliable methods of doing it.
