Not a cat owner, but I know others with this sort of problem and what worked for Gabe is having one more box than cats (so three) and having one box in a different location.

Good luck.

We still have a few issues with my cat but locking her up in a bathroom at night with her own box helps a lot. There is also a pheremone scent thingy that I'm about to try to be a calming influence on the cats. Once Benton started crawling we started having issues.

My sofa cushion with legs did much the same, and truthfully I just kept her in a single room to minimize the damage - Marta wasn't exactly right in the head - so I'm no help at all. I'm assuming that since there were vet visits you've ruled out kitty diabetes? I never figured out which was egg and which was chicken in our case. Not all overweight cats fail at litter usage, but I imagine it's a contributing factor. Sorry I can't be more than sympathetic :(

Is it only happening when the litter box is dirty? Sometimes particularly prissy cats won't use a litter box if it gets too dirty. Maybe a litter pad outside the litter box designed to get the litter off the claws? Is there somewhere to put the litter box on tile?
