Looking for Cat advice

Some of you know this has been a long running problem. We've tried a lot of things, looked a lot on the web, and yet still we have an issue with one cat. (This is a two cat house)

So I'm looking for open commentary. Issue: Said cat intermittently doesn't use the box. Mostly poop on the floor, but will also shift to pee on the floor. Always (so far, cross your fingers!) in the mud room with the cat boxes, where the carpeted floor is actually carpet tiles that can easily be pulled up and washed.

I live in fear for the day that the damage goes beyond the mud room.

Anyhow. Remedies tried: Cat to vet for UTI. Only a modicum of success. Lids removed from boxes. Litter brand changed twice. Box configuration changed. Two boxes. Larger boxes (this is a jumbo sized cat). "Kitty prozac collar" added. Nightlight added. Two separately located free flowing water dishes in the house. Multiple food dishes.

The most lasting solution lately was the 1 week stay at the vet in June. She behaved herself for nearly a month after.

The latest round started when Christmas started to go up and a tree appeared. No sign of needles in stool, urine smells like cat pee.

She's a major league jumbo cat, and the boxes are underneath an area. We haven't explored pulling the boxes fully into the open yet, which will be a significant challenge given the floor space available.

I'm looking for thoughts of other things to try, or at least theories on possible causes. Thank you!
