Am about to read all comments from last night but -- I just realized that the only story actually name-checked in the special had a twin theme!

Wasn't there a canon story with triplets somewhere?

And incidentally, the biggest argument I can think of for shooting down (you should forgive the pun) the "he shot a second gun into the ground" scenario is ... smell. It's never mentioned, but if someone blows their brains out, you can smell brains and blood. Kind of hard to fake that. (Also, Moriarty could not have used a blank to shoot himself in the mouth. That would have killed him just as dead as live ammunition would.)

I'm thinking that blood wasn't that hard to get in a world with blood pudding. Or Mollystache collected the effluvia of blowing out the lookalike corpse's head.

But in that case, Sherlock would have been able to smell it *before* the gun was fired. What I'm talking about is the shock of the gunshot and the immediate scent of fresh blood which wouldn't have been there before. (I'm also thinking more about the rooftop scene. Where would Moriarty have a) hidden a pot of blood that b) he could trigger at the right time? I mean, an exploding bladder on the back of his head? Really?)
