For my eczema, I tend to use oatmeal masks, and moisturize either with olive oil or with Burt's Bees Milk & Honey lotion (the old version, which is less all-natural than the new one, but works better on my skin).

Maybe you should add a hydrator to your regimen? Something like hyaluronic acid cuz you seem to be moisturizing to compensate for your dryness without hydrating your skin first. I would suggest finding a good hyaluronic acid product to use as a base to your moisturizer INSTEAD of grape seed oil.
The only problem with this is trying to find an all natural hyaluronic acid product. 100percentpure (my favorite all natural line) USED to sell one that was just rose water and hyaluronic acid but they discontinued it (and I was pissed). Hada Labo Super Hyaluronic Acid is a really good product but it's not all natural (the ingredients aren't bad though). It has a very thin watery/gel consistency and once it absorbs seal that hydration in with your moisturizer.

Continue drinking water and I hope this helps.
