John stares blankly at Ianto for a moment, then slowly recognition dawns in his eyes. He lunges at Ianto and grabs his arm very tightly. Breathing heavily, he croaks, "I'm here. I'm here." He tries to grab Ianto's other arm, but flinches with pain. He has a big bandage tied around a gash in his upper left arm. Another one tied around his left hand. It has a big blood stain where his thumb used to be.
"You're here," Ianto says, trying to keep John steady and taking in every detail with increasing concern. "Your hand--Christ, what happened to you? Let me get you to the infirmary."
John looks at his hand as if he's just remembered he has it. "It had to come off," he says like it's the most natural thing in the world. "Gi'us a fag, yeah?"
There was a man in Starfleet blues peaceably drinking a glass of bourbon at the Bar, who was contemplating whether he was going back to the ship tonight or not.
Now there's a man in Starfleet blues crouched next to John, and to be very honest, that man doesn't look particularly happy.
"Good God man, is there a reason you haven't gone to the infirmary, or a damned doctor on your side of the door, or has a concussion utterly scrambled your brains?" McCoy saves his good bedside manner for those that have earned it. Everyone else gets scolded.

Which is why she slips through the bar, footsteps quick and silent, and stops next to Constantine.
"John."
Beat.
"You are not okay."
It would be a question, but -- some things are obvious.
