The most obvious indicator is the fact the shower is running; James's coat and keys are abandoned in the living room, and he doesn't seem to have been home long.
(It is worth noting that evidently while they were both out, his housekeeper was in.)

Pelagia obviously sees no reason to knock or wait for him to get out of the shower; she has no clothes on, she can, presumably, join him just as easily. The water would be nice, anyway.
However, because she's not stupid, her footsteps aren't quiet on the floor and she doesn't disguise her presence. There are people you don't want to surprise too terribly much in their own home, even if you're a warrior woman like Pela, and James Bond is one of them.

Particularly when you have a knack for agitating him in the first place, yes, that would be classified a bad idea. James, with his head under the water and his hands on the wall, doesn't take as long as someone else might to realize that he's not alone.

Pelagia steps inside, leaning on the wall behind the water--it's a little chilly for most people there, but she doesn't really feel the cold like everyone else does. Not today, anyway, with some of her glamour restored.
