I'm in the camp of believing Reid's being sarcastic here. I just don't buy that Reid would ever say something so melodramatically over-the-top as "you'll pretty much be guaranteeing that he will NEVER SEE AGAIN," unless he's making a dig at someone who has a habit of being melodramatic/over-the-top, and on top of that, I really hear a note of sarcasm/irony in the way he says "never see again."
I agree that it makes no sense whatsoever for Reid in one episode to be asking Luke if he's made it clear to Noah that they're broken up (and being upset when that obviously isn't the case), and in the next to be saying Noah's fragile brain can't handle the truth.
I suppose I could be wrong, but at the moment, I'm not concerned at all, and I think it's just that the preview is trying to mess with our heads.
Still, I wouldn't object to some seriously good fanfic myself....
Well, there's a difference between Noah accepting the truth that he and Luke have broken up, with no getting back together- and being able to accept that Luke has moved on with someone else. The first is much less stressful than the first and second added together.
Plus there have been hints that there may still be some problems with Noah's surgery. So I really do think that Reid is trying to be a good doctor and put the patient's needs above his own desires.
I've already had to suffer a failboat of an SPN finale, please don't fuck this up ATWT writers. Won't somebody please think of the children fangirls. D:
From personal experience preview clips and spoilers can often be very misleading, and edited out of context to a ridiculous degree. The clip could be taken in several different ways, 10 seconds of dialogue isn't a lot to go on.
Your concerns aren't baseless but I would definitely try not to worry too much before you've seen the whole episode. :) IMHO this would tie in with the spoiler "Reid takes the high road" in that he's putting Noah's health before his own feelings (because he's such an awesome doctor.)

We have it on good authority from a member of the ATWT boards that it is Luke who is reticent to tell Noah, and in this scene, Reid is just repeating Luke's rationale to him. Reid wants him to tell Noah. So, I wouldn't worry too much if I were you on this point...
The only way I could see Reid not wanting to tell Noah, is because he knows it would be a health risk, or he was just being sarcastic. Reid the doctor, has to put Noahs health first. But Reid the man, couldn't care less about Noahs feelings. I hope it was just Reid in doctor mode.
