This happened to my mom when the social security / permanent disability office sent her for a pysch. evaluation. (Because for some reason the confirmed diagnosis of multiple sclerosis and left side body paralysis wasn't enough proof:P )
The doctor gave her the inkblot testing and she kept replying things like "A monarch butterfly" or "One of those huge butterflies you'd see in a science museum under glass."
He finally told her "It's is okay for you to answer honestly, and not force yourself to reply with positive answers such as a harmless butterfly."
She said "All your cards look like black & white watercolors of butterflies...sorry, but I like butterflies so maybe that is why my brain keeps telling me that it's butterflies in your blots..."

I think there's also supposed to be a right, or most common, answer. But I'm not sure what it is.
I see a few different things but nothing that's consistent across the entire image.

Looks like a demon/gremlin type creature to me. The first word that came to my mind without even thinking was "Gremlin"
