You can't create a post form as a provisional user on the site, because you can't create the behind the scenes pages required. Something about a .asp page to cache the answers you receive. I get about that far and then throw my hands up and move on with forms every. single. time. Either way, that would be why the "This form is inoperational!" error is coming up for you.
