I know that in the body mod community the effect of having horns is created with silicone implants under the skin. You start off with a small implant which stretches the skin a little, then gradually replace it with larger and larger ones stretching the skin more and more, until you get the size you want.
Try searching on body modification and horns. There are some pretty explicit pictures of the whole thing.

Go and read about the BAHA bone-anchored hearing aid system. This passes sound through the skull using an osseointegrated implant of a type that might reasonably be used for this sort of thing (it's also used for prosthetic ears).
That's not what I'm referring to, though; the intention was for the skin to be peeled back and items bolted to the skull without being under the skin. Sorry if I didn't explain it well.
Could be something like peeling the skin back and setting the base of the horn against the skull, perhaps with a very small pin, like a thumbtack, to hold it in place. It could have a sharp edge, like a saw, and could be rotated to grind it a short way into the bone. Apply current (or magic or whatever) and phlanges deploy horizontally from the base edge, lodging in the bone. The phlanges wouldn't need to be very long, maybe a quarter inch or so, and if you want to be *really* gruesome, put some backward-pointing spikes on them so they can't easily be worked out.
