Hell, they were still doing that in the (early) 90s! My parents got the "Either you put her on medication or she's getting expelled" ultimatum from my elementary school, and it took sessions with two different child psychs to prove that I was highly gifted, not ADHD.
I believe the previous designator for AD(H)D was "Minimal Brain Damage".
Various things that I know can be mistaken for ADHD include but aren't limited to:
-Bipolar Disorder (and this is a very BAD mistake to make, since in many cases stimulants will make the bipolar worse). - Minor hearing impairments (kid who seems to not be paying attention might actually not hear

ADD can occur in people with bipolar disorder as well. Stimulants can be used if other medications are already in place to control the bipolar symptoms, although careful monitoring will have to take place. I have both and do fine.
That still happens. ADHD is not a clearly defined diagnosis and it's not unusual at all for kids to be thrown on Ritalin if they start to act up. One of my sister's childhood friends was on Ritalin for over a decade before they actually diagnosed him with something - a form of petit mal epilepsy.
I grew up in the 1960s and 1970s, and as far as I remember back then kids who would now be classed as ADHD would have been called "poorly parented", "antisocial", or even just plain "bad

A bit tangential, but I know Ritalin was easier to get a hold of. My mother said that back when she was a blood tech in the late 60s and the 1970s, they'd take Ritalin out of the storage cabinet and pop one if they needed to pull an all-nighter. Everyone knew, and no one cared.
I also know adults (non-ADHD) who decided one day it'd be fun to take Ritalin as a recreational drug. They were up all night cleaning. One called his evil father and tried to bury the hatchet. When they came down, they were all severely depressed, and one had suicidal thoughts :/

Exhaustive information on Ritalin. This site points out that you start with small doses and build up over time (like most psychological drugs). It's possible in that case the "high" effects wouldn't be so noticeable, and even if they were people might continue because they're convinced the higher doses will squash the effects.
