I so, so agree with this summary. One of the ugliest facets of the healthcare debate is those who enjoy 'good health' convincing themselves that it is a just and fair outcome of their superior lifestyle choices, and those who don't have only themselves to blame. And therefore we don't really have to help them. Puritanism + class entitlement to the max.
