I can't figure out what that smooth mass is either, but I agree with the pregnant assessment.
It is a bonnet--albeit an early shape, and a rather ugly one that doesn't seem to show up too much at events. I think the caul part might be her cloak collar with the tie in a bow. It also appears she has the "mob cap" thing going on, with the cap tied under her chin.
Yeah, you're probably right that that is her cape collar. I want to say I'm getting an optical illusion from the bonnet, but if you follow the line of the edge of the brim, it appears to cross over where her neck would be, rather than go behind it, if that makes sense. I guess my brain is just conflating the top edge of the collar with the bottom edge of the brim. I can't unsee it!
And yes, that is a particularly unflattering style.
