I resorted to using a big wedge pillow to keep heartburn at bay when sleeping.
It's 1ft high at peak wedge, and 3ft long, and about 3ft wide.
So, my body is angled upwards when I sleep, from the hip, and for me, seems to have defeated waking up with pain, as gravity keeps things away from my esophagus.
Sleeping on the wedge on my stomach seems to work best. When I sleep on my side, I tend to 'slide down' and wake up with the heartburn.
Might help, if you have not added it to your 'things to try.'

The other strategies I do that might help (if you are not doing them already) is:
that I avoid eating any food or snacks about 2hrs before bedtime, and have a 1/2 glass of water at bedtime (dilution strategy), even it it makes me wake up to go use the bathroom, all in attempts to keep stomach not actively digesting during bedtime.
